I understand there are obvious known dangers/risks inherent in participation in an Innovative Choreography event, including but not limited to injuries sustained through a fall or loss of personal property, and I voluntarily agree to assume such risks.  I know that any physical activity presents certain dangers of mild, moderate, or severe injury.  I acknowledge that Innovative Choreography, and their instructors/choreographers do not have control over these dangers.  I will follow the directions of camp officials/choreographers and do my best to reduce risks by using proper techniques in play and conditioning and following the advice of the camp official/choreographers concerning the prevention and treatment of injuries and illnesses.

In consideration of Innovative Choreography permitting my participation in the event, based on my reputation that I am in proper physical condition to participate, I agree:

1.  To assume all risk of injury to myself and risk damage to or loss of my property arising from my participation in the event.

2.  To release and forever discharge Innovative Choreography and their instructors/choreographers from any and all claims or liability for any injury, including death, and for property damage or loss which may be suffered by me arising out of or in any connection with my participation in the event..

3.  Myself, my heirs, executors, administrators, and coaches/teachers assign to indemnify and hold harmless Innovative Choreography and their instructors/choreographers from any and all liability, claims, demands, actions, loss and damage arising out of my participation.

I/WE HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS.  I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN INNOVATIVE CHOREOGRAPHY AND MYSELF, AND I SIGN OF MY OWN FREE WILL.

I release all rights to photographs and videos to be used in, but not limited to, art, advertising or website without any compensation or recognition. All forms become the property of Innovative Choreography.

I waive my right to proof, alter, or restrict use of the finished product.

I release Innovative Choreography of any responsibility for any injuries occurred during the photo/video shoot.
